本研究主要目的在探討乳癌婦女性生活現況及影響性生活滿意度的相關因素。研究方法以立意取樣(purposively sampling),採橫斷性研究設計,以台灣北部某乳癌病友支持團體成員為對象,收案時間為2020年3月17日至3月31日,有效樣本數101份。採結構式問卷進行調查,收集乳癌婦女社會人口學、乳癌疾病特性、症狀經驗、身體心像、性伴侶親密關係及性生活滿意度等相關資料,使用SPSS for Window 23.0版統計套裝軟體進行描述性及推論性統計分析,以了解各變項之現況以及影響性生活滿意度的相關因素,並探討性生活滿意度的預測因子。本研究結果歸納如下:
一、 研究對象的身體心像滿意程度屬於中等偏上,以「滿意自己身體外觀」為最高分。性伴侶親密關係程度屬於中等偏低,以「我們倆嘗試一起參加社交活動」為最高分。性生活滿意度是中等略偏低,以反向題「你感覺性吸引力減少是因為疾病造成的外觀改變」為最高分。
二、 研究對象的社會人口學變項,年齡和性生活滿意度是顯著負相關;社經地位指數和性生活滿意度是顯著正相關;已婚較其他婚姻狀況的性生活滿意度為差。
三、 研究對象的乳癌疾病特性,以「有使用活動式義乳」較「無使用活動式義乳」的性生活滿意度為高,其他變項則不顯著。
四、 研究對象的症狀經驗,以「有月經」較「已停經」、以「自覺月經週期有受到治療的影響」較「自覺月經週期無受到治療的影響」的性生活滿意度高;「自覺心理症狀數」和性生活滿意度是顯著負相關。「身體心像」及「性伴侶親密關係」和性生活滿意度是顯著正相關。
五、 預測乳癌婦女性生活滿意度的因素為婚姻狀況、使用活動式義乳及性伴侶親密關係。
六、 研究對象之社會人口學、乳癌疾病特性、症狀經驗、身體心像、性伴侶親密關係對預測性生活滿意度,可解釋的總變異量為71.8%。
The primary aims of this study are to explore the sexual life of women with breast cancer, and the factors that affect those women’s sexual satisfaction. This research adopted purposive sampling and cross-sectional study on a patient supporting group for women with breast cancer in northern Taiwan from March 17 to March 31, 2020. In total, 101 effective questionnaires were collected. These were structured questionnaires that collect data on the socio-demography of the women with breast cancer, the disease characteristics of breast cancer, the symptom experience, the body image, the sexual intimacy with their partners, and the satisfaction with their sexual life. This research used SPSS for Window 23.0 statistical package software to present descriptive and inferential statistical analyses. By doing so, this research seeks to understand the variables mentioned above and the relational factors that affect the sexual life satisfaction of women with breast cancer, and to explore the predictors of their satisfaction with sexual life. The results of this study are summarized as follows:
1. The research subjects’ satisfaction of their body image is above average, and the "satisfaction with their body appearance" has the highest scores. The degree of intimacy with their sexual partners is medium to low, and the "couples trying to participate in social activities together" has the highest score. Their satisfaction with sexual life is moderately low, and the question: "You feel that your sexual attraction is reduced due to the changes in appearance caused by the disease" has the highest score.
2. Regarding the sociodemographic variables of the research subjects, it is found that the correlation between age and sexual life satisfaction is significantly negative. However, there is a significantly positive correlation between socioeconomic status index and sexual life satisfaction. The responses show that married couples are less satisfied with their sexual life than other relationship statuses.
3. In relation to the disease characteristics of breast cancer among the research subjects, the sexual life satisfaction of the women "with breast prosathesis" is higher than the ones "without breast prosathesis". However, other variables do not show significance.
4. When responding to the question of symptom experience, the respondents who have "menstruation" show higher sexual life satisfaction than the ones who have experienced "menopause". It is also found that the respondents whose menstrual cycles have been affected by the treatments feels higher sexual life satisfaction than those who feel their menstrual cycles are not affected by the treatments. The correlation between "number of psychological symptoms" and sexual life satisfaction is significantly negative. But, there are positive correlations between "body image" and sexual life satisfaction, and between " intimacy with sexual partner" and sexual life satisfaction.
5. The factors to predict the sexual life satisfaction of women with breast cancer include marital status, the use of breast prosathesis and intimacy with sexual partner.
6. The sociodemographic, disease characteristics of breast cancer, symptom experience, body image, and intimacy with sexual partners for the research subjects can be the factors to predict the sexual life satisfaction of breast cancer women. The explained variation is 71.8%.
