本研究旨在探討新住民罹癌病患的心理調適歷程,並進一步探討文化如何成為調適歷程之影響因素。 本研究使用敘事研究,共有兩名罹癌新住民為研究參與者,分別來自印尼與中國。透過半結構的個別訪談蒐集資料,並使用敘事分析法「整體-內容」進行資料分析,來理解兩位受訪者的心理調適歷程及背後有關文化社會影響因素。透過兩位受訪者的生命敘說與敘事分析,有兩個主要研究發現:
一、兩位受訪者的心理調適歷程都經歷診斷期、治療期以及適應期三個階段,但兩人在每個時期的心理與情緒反應不盡相同;二、新住民罹癌心理調適歷程受到個人、家庭以及社會三個系統的影響。個人系統包含受訪者罹癌的疾病階段 、人格特質、以及對文化差異的適應。家庭系統包含在台家庭關係與家庭支持系統。社會系統包含宗教的力量、社會階層以及社會支持。這三個系統內的各個元素都各自會影響受訪者的罹癌心理調適,而這三個系統也會彼此互相影響受訪者的心理調適歷程。最後,研究者依據本研究結果作出討論,並針對本研究限制、未來研究方向、醫療單位、諮商教育與實務工作者以及台灣政府提出建議。
The purposes of this research were to explore the psychological adjustment process of new immigrant cancer patients and to reveal the cultural factors that influenced the adjustment process. The researcher adopted narrative research approach and conducted individual interview to two new immigrants cancer patients from Indonesia and China respectively. Data was collected using semi-structured interviews and analyzed with “holistic-content approach” method to explore the process of psychological adjustment and the influence of cultural factors of the adjustment process from the two interviewed data. Through life narratives and analysis of the two interviewees’ data, two major results of this research were obtained as follows: 1. The psychological adjustment process was divided into diagnosis period, treatment period, and adaptation period. These two research participants showed somewhat different emotional and psychological reactions in each of the three periods. 2. Factors influenced the psychological process came from three systems, namely individual, family and societal system. Individual system included patients’ cancer stages and the impact of the disease, personality traits, and the adaptation to the new immigrant culture. Family system included family relationship and family support system. Societal system included the religious/spiritual strength, social class, and social support. Lastly, the researcher discussed the research findings and research limitations. Researcher also provided suggestions to future researchers, medical units, counseling educators and practitioners, as well as Taiwan government.
