  • 本研究採隨機、雙盲、有安慰劑對照的並行試驗,招募90位45-60歲停經早期、血清FSH大於40m IU/ml的健康婦女,隨機分兩組,一組為異黃酮加鈣組(n = 62),每天早晚空腹服用兩次黃豆異黃酮 (120mg/d) 與鈣 (560 mg/d),另一組為鈣組 (n = 28) 服用同量安慰劑(無糖麵茶)與鈣。實驗前、服用3與6個月後收集血與尿。以尿液異黃酮排出量評估受試者配合度,並依尿液中是否有equol,將異黃酮加鈣組再區分為能產生equol者 (n = 31) 與不能產生equol者 (n = 31)。以Kupperman index評估更年期症狀改善之效果,雙光能吸收儀(DEXA)測量骨質密度,並比較血脂、C–reactive protein (CRP)、雌二醇(E2)、濾泡刺激素(FSH)、性激素結合蛋白(SHBG)、骨質合成指標-血清osteocalcin(OC),骨質分解指標C-telopeptide of type I collagen (CTX),與尿液骨質分解指標deoxypyridinoline (DPD)。結果顯示,異黃酮加鈣組對Kupperman index的改善與安慰劑加鈣組無顯著差異,但其中能產生equol者補充異黃酮加鈣後,對臉部潮紅、容易疲倦及Kupperman index 總分的改善顯著較佳(p<0.05)。血脂、CRP、FSH、LH、SHBG皆無顯著差異。異黃酮加鈣組E2顯著上升(p<0.05),其中不能產生equol者的E2濃度顯著最高,且有顯著最低的DPD(p<0.05)。安慰劑加鈣組的OC與DPD都顯著高於異黃酮加鈣組(p<0.05),且腰椎骨及左、右臗關骨骨質密度顯著優於異黃酮加鈣組(p<0.05)。綜上所述,補充鈣6個月使骨質轉換速率及骨密高於補充異黃酮加鈣;不能產生equol者補充異黃酮加鈣可顯著上升血清E2濃度且顯著下降骨質分解;能產生equol者補充異黃酮加鈣可顯著改善更年期症狀。故補充鈣或異黃酮加鈣對停經婦女各有不同的助益,個人是否能將異黃酮代謝產生equol也會影響異黃酮的功效。
