本研究目的為探討北區某醫學中心兒科護理人員罕見疾病知識、罕見疾病教育態度與在職教育需求。根據罕見疾病基金會所提供之行政院衛生署國民健康局認證通過之遺傳諮詢中心名單,選取北區醫學中心台大醫院之兒科及基因醫學門診、兒科病房、新生兒及小兒加護病房、嬰兒室護士,且於該單位工作年資(含)半年以上作為研究對象,有效樣本人數為127人,回收率63.5%。
資料收集方式先以個別訪談進行初步質性資料蒐集,主要瞭解護理人員對罕見疾病的教育需求,以作為發展本研究之結構式問卷初稿,並徵詢相關領域專家學者之意見,進行問卷前驅性施測(pilot test)之後,針對問卷信效度加以修改並進行施測及分析。本研究主要結果如下:
一、兒科護理人員之罕見疾病知識偏低,以「罕見疾病基本概念」得分最高,「遺傳模式與症狀」分數最低。與罕見疾病知識得分之相關因素為:是否在職進修、單位服務年資、職位、有無參加罕見疾病之相關課程研習、訊息來源為「報紙、雜誌或電視、廣播」、「教科書或專業期刊」等變項。
二、兒科護理人員之罕見疾病教育態度相當正向。以「罕見疾病教育的接受度」得分最高,最低為「責任意識」。與罕見疾病教育態度之相關因素為:年齡、是否在職進修、護理總年資、單位服務年資等變項。
三、兒科護理人員之罕見疾病在職教育需求高。需求最高的課程內容為「遺傳及罕見疾病照護」;最喜歡的教學方式為:「直接授課」;參與在職教育的誘因是「公務人員時數認證」。與在職教育需求相關的變項為年齡、護理總年資、單位服務年資。
四、兒科護理人員之罕見疾病知識與罕見疾病教育態度無統計上顯著相關;而罕見疾病知識與罕見疾病在職教育需求在統計上呈現低度正相關,可解釋的總變異量為3.61%;罕見疾病教育態度與罕見疾病在職教育需求具有高度的正相關,可解釋的總變異量為46.7%。
本研究成果可作為在職教育訓練課程之參考,亦提供學術研究的後續探討、教材研發以及未來教育介入之參考。
